paper
Step-1:

In this tutorial we are going to attempt to draw a page from a notebook. Start a document 400x400 and fill it with whatever color you want the background to be. I used gray. Note the icon that is circled_ It should be selected for the next steps.

Step-2:

Create a new layer Layer-New-Layer. Now lets draw the shape of the paper using the Rectangular Marquee Tool and lets fill it white.

Step-3:

To make things easier lets do View-Show-Grid.

Step-4:

Select the Single Row Marquee Tool. Create a new layer Layer-New-Layer.

Step-5:

And start drawing the lines in the paper. Use the background to decide how far apart the lines should be. All you have to do is touch the line.

Step-6:

Using the bucket tool, choose a shade of blue and fill the layer. When you do it, you wont notice anything but it will do its job.

Step-7:

Create a new layer Layer-New-Layer and in it, select the Single Column Marquee Tool.

Step-8:

Draw the red lines on the left and right of your drawing and using the bucket tool, fill the layer with Red. Again, you wont see any changes.

Step-9:

Now select ONLY the layers that have the vertical and horizontal lines. Turn everything else off. Look at the picture for guidance. Once the layers are merged, lower the opacity to your liking.

Step-10:

Click on Layer-Merge Visible.

Step-11:

Now turn everything off and leave only the paper layer. Now select the Magic Wand and touch the center of the paper to select it.

Step-12:

Click the layer with the lines now and click on Select-Inverse.

Step-13:

Click On Edit-Cut.

Step-14:

And there is your paper. If you notice that your lines and the paper are of different size, go to Edit-Free Transform-Scale and correct one of the layers to match the other.

Step-15:

You can repeat the process using different colors. Do not forget to draw the holes on the left.
